Had better is similar to a modal auxiliary. It doesn't change its' form for tense or person etc., and is followed by the basic form of the verb.

Had better is used for recommending action or giving advice.

In informal speech, it's usually reduced to ( 'd better) or just better.

Structure

1. Positive

subject 'd / had better verb (basic form)
You had better go shopping this weekend.
We had better stop smoking.

People who say I'd better or We'd better are offering or giving advice to themselves. Example: We'd better complete this job before 4:00pm so that we can leave early.

2. Negative

subject 'd / had better not verb (basic form)
We 'd / had better not make her cry.
You 'd / had better not come home.

We can use short form like: You'd better not come home. or You better not come home.

3. Question and Negative Question

Because had better is used for recommending action or giving advice, so it's rarely used in Question.
Example:
Had you better go shopping this weekend? (this is rare)

In Negative Questions, we add n't to had.
Example:
Hadn't you better come home? (this is also rare)

Note: We also add n't to had in Negative Tag Questions. Example: You'd better go shopping this weekend, hadn't you?
